Eligibility Requirements

At Social Day Journeys, we strive to create a safe, supportive, and engaging environment for every participant. To ensure we’re able to meet the needs of those we serve, we’ve outlined a few basic eligibility requirements. These guidelines help us provide the best care possible while maintaining a comfortable and enriching space for all. Please review the criteria below to see if our program is the right fit for you or your loved one.

Couple Scrapbooking

We welcome participants who meet one or more of the following criteria:

  • Require the assistance of another person for at least one Activity of Daily Living (ADL), such as toileting, mobility, transferring, or eating.

 

  • Require supervision due to cognitive and/or psychosocial impairment.

 

  • Are adults aged 55 and older who do not require daily assistance but are looking for social engagement, community activities, and companionship.


 

  1. Age & Residency Requirements 

 

- Must be at least 60 years old  

 

- Must be a resident of New York State or currently residing with a caregiver who lives in New York State.

 

2. Health & Functional Status

 

- Must be ambulatory or semi-ambulatory

 (can use a cane, walker, or wheelchair but must not require total assistance).  

 

- Must be able to perform basic self-care tasks (or have a caregiver assist them if needed).  

 

- Must not require 24/7 medical supervision.

 

- Cognitively able to participate in group activities (individuals with mild cognitive impairment may be accepted).


 

3. Behavioral & Social Requirements  

 

- Must be able to engage in a group setting without disruptive behaviors.  

 

- No history of aggression, violence, or severe behavioral issues that could pose a risk to others.  

 

- Must be able to follow basic safety instructions from staff or be guided/directed


 

4. Medical Clearance & Emergency Protocol

 

- Must complete a health assessment form from a doctor (confirming ability to participate). 

 

If you are without a Primary Care Physician our In-house Nurse Practitioner can perform initial and on-going assessments.

 

- Must have filled out and completed the Enrollment Package.

 

- Must disclose any dietary restrictions, allergies, or special needs for accommodations.

We Accept MVP, Anthems, UHC all plans, The Empire Plan, Medicaid, Medicare, Fidelis, Blue Cross Blue Shield, Health first, First Health.

 

All registration forms, enrollment packages, financial obligations and participant agreements must be signed and completed before program participation
